Where Your Scars Are Written

A skin graft shrinks the moment you lift it, and the thicker the graft, the more it shrinks. Which sits very awkwardly beside the thing everyone is taught, that thicker grafts contract less. Both are true. They are two different contractions, running in opposite directions, and separating them is the difference between a full answer and half of one.

On the face, elective incisions are laid along the relaxed skin tension lines; on the digits, no incision may cross a flexor crease at a right angle. Which single property of scar tissue do both rules exist to manage?

  • AA scar does not regain the full strength of the skin around it, so it must lie where tension is least
  • BA scar cannot lengthen, so its axis must not lie along a direction the skin has to stretch
  • CA scar contracts only during its first months, after which splintage ceases to matter
  • DA scar recovers its elasticity as elastin returns during remodelling, provided tension is avoided
  • ECollagen in a scar aligns with the underlying muscle fibres, whatever the incision’s direction
