The outermost layer of your body has no blood supply at all. None. Everything the epidermis receives, it receives by diffusion from a single plexus in the dermis beneath it, which means the quality of your dermal closure decides whether the layer above it lives or dies.

Anatomy is poorly taught and often treated as a tickbox exercise. Crammed for examinations, reproduced on diagrams, and quietly forgotten. The forgetting has consequences, and nowhere is that more apparent than in the largest organ in the body.

In this first episode, Dev explores the skin as a surgically applied subject: the avascular epidermis, the vascular architecture of the dermis, and the story of how Ian Taylor's angiosome concept transformed reconstructive surgery in 1987. Understanding it begins not in the operating theatre but here, with the anatomy that was always worth knowing properly.

Over most of the body the skin can be pinched up and slid freely, yet in a few regions it is bound down and barely moves; and when a deep lesion later tethers mobile skin, displacing the skin produces a telltale dimple. Which structural system accounts for both observations?
